HOW TO ESTABLISH CARE AS A NEW PATIENT WITH DMC :

Fill out and submit a medical records release form (prefilled forms for specific providers are below or you can click HERE for a generic form).

Check with your insurance plan to see if they require you to update your Primary Care Physician in their system.
