[DMC at Home] Jaclyn Vesey became a registered nurse (RN) through NHTI in Concord, NH. She earned her  Bachelor’s degree in Nursing from Franklin Pierce University, and her Master’s degree in Nursing from Massachusetts College of Pharmacy and Health Sciences. Jaclyn is a Board-certified Family Nurse Practitioner through the American Association of Nurse Practitioners.

Working with older adults has been the focus (and joy) of Jaclyn’s career since the beginning.  She believes in empowering her patients in the clinical decision-making process. Jaclyn promotes open communication about how evidence-based science applies to health management. She enjoys collaborating with her patients to help them meet their goals, while supporting their independence, dignity, and quality of life.

Jaclyn views the provider-patient relationship as a partnership, in which patients feel heard and supported in their healthcare journey.

When Jaclyn is not seeing patients, she can usually be found outdoors doing any variety of DIY projects for their house or cars, and spending time with her family, including the cats and dogs.

Jaclyn sees patients in the DMC at Home program. She works closely with other DMC providers as part of her patients’ care team.
