How the Care Management Program Works

It can be difficult to manage more than one medical condition. It often involves numerous trips to the doctor plus many different medications, each with special instructions.

The Care Management Program is here to help! It starts with a referral from your primary care provider. You will be assigned a dedicated Care Manager who will review your overall health and create a personalized care plan to meet your healthcare goals. The Care Manager, supported by a Care Coordinator, will closely monitor your progress, typically over the phone.

At the center of the program is the Personalized Care Plan, developed by you, your provider and your health care team, to help you manage your medical conditions. Enhanced care coordination, a focus on your goals, and quality of life are at the heart of the program.

A DMC Primary Care Care Manager is with you every step of the way to:

  • Coordinate with your primary care provider
  • Access resources in the community such as pharmacy, mental health, and transportation
  • Assist with medication review and management
  • Follow-up after hospital, emergency room or urgent care visits
  • Coordinate care with specialists
  • Reduce your risk of complications through healthier lifestyle choices
  • Provide education through health coaching and shared decision making
  • Address any symptoms or barriers to improved health

Frequently Asked Questions

Will this program affect my insurance coverage?
No, the Care Management Program is not an insurance program. Your provider will encourage you to participate but you are not obligated to do so. Whatever you decide, it will not change your current health benefits.

Who will manage my care?
Your primary care provider will continue to manage your healthcare. The relationship you have with your primary care providers and other healthcare specialists will stay intact. The Care Manager and Care Coordinator are part of the DMC Primary Care team, and are there to help you.

How are patients selected to participate?
Patients must meet certain requirements to participate in the program. Selection is based on a recommendation by your provider, and the anticipated benefit to you.

What services are available in the program?
A variety of services are available depending on your personal needs. You will have a personalized treatment plan developed just for you. It may include phone calls from your Care Manager or Care Coordinator, identification of community resources, and assistance with issues related to helping you achieve better health and wellness. You will continue to have access to all the resources and treatment options available at DMC Primary Care.
