New Patient Information

WE ARE SO HAPPY YOU ARE JOINING THE DMC FAMILY

ESTABLISHING CARE IS EASY:

1. Please call our Patient Access Center at 603-537-1300 to be registered as a new patient or CLICK HERE to self-schedule your first physical exam online. Please be sure to have your insurance information on hand when calling or self-scheduling.

2. Please fill out the documents in the New Patient Packet (below) right away so that your previous medical records can be sent to DMC prior to your first appointment with us.

3. Check with your insurance plan to see if they require you to update your Primary Care Physician in their system.
