Sara Carrier, PA-C

[Concord Office] Sara earned her Associate of Science degree in Physical Therapy from Becker College, Worcester MA. She earned her Bachelor of Science degree in biology from Notre Dame College, Manchester, NH and her Master of Physician Assistant Studies from Massachusetts College of Pharmacy and Health Sciences, Manchester NH.

in addition to routine preventative care, Sara enjoys working with newborns and helping new moms through the breastfeeding challenges that sometimes occur.  She also performs minor surgical procedures as well as joint and trigger point injections.  

“What I enjoy most about family practice is I am able to treat the person as a whole.  I can address and help with issues of mind, body and spirit as they are frequently intertwined in illness.  It is a privilege to take care of multiple generations in families.  I am always humbled and honored when a long-time patient brings me their newborn to care for.  I treat my patients like they are family, and many really are.  I strive to help my patients live the best life they can.”

 Sara enjoys spending time with her husband and three boys.  If she’s not in a hockey rink, you’ll find her out on the lake, hiking on local trails with her yellow lab, planting flowers, or finding a need for a new power tool! She is not a fan of winter, but after spending 16 + years in hockey rinks, she jokes that she has learned to dress for the elements, embrace it and find joy. You’ll even occasionally find Sara on the ski slopes.

Patrick Fox, MD

[Concord Office]  Dr. Fox earned a Bachelor of Science degree in Nutrition from University of Florida, Gainesville, FL and a Master of Science degree in Nutrition from Boston University, Boston, MA. He earned his Doctor of Medicine (MD) degree from Eastern Virginia Medical School, Norfolk, VA. He completed his residency at NH Dartmouth Family Medicine in Concord, NH.

Dr. Fox enjoys the full spectrum of family medicine—caring for patients and their families from birth to death. He feels it is important to work as a teammate with his patients to help them live a healthier life.

Jill M Waren, MD

[Goffstown Office] Dr. Waren earned a BS in Microbiology from Pennsylvania State University, State College, PA and her Doctor of Medicine (MD) degree from Temple University School of Medicine, Philadelphia, PA. She completed her family medicine residency at Beverly Hospital, Montebello, CA.

Michael Marolda, MD

[Derry and Raymond Offices] Dr. Marolda earned a Bachelor of Science degree in Biomedical Engineering from The George Washington University in Washington DC. He then went on to earn his medical degree from the University of Queensland in Australia prior to returning to New England to complete his residency at NH Dartmouth Family Medicine in Concord NH. He is board certified in Family Medicine.

Before joining DMC, Dr Marolda worked in hospital medicine and has a special interest in geriatrics and managing chronic conditions such as diabetes and heart failure. He enjoys taking care of patients of all ages; from pediatrics to seniors.

In his free time, he enjoys running, golfing and hiking with his partner and rescue silver lab named Gracie. He is currently learning the fastest growing sport in America: pickleball!

J. Bryan Bannister, MD

[Bedford Office] Dr. Bannister earned his bachelor’s and master’s degrees from Texas Tech University, Lubbock, TX and his Doctorate in Medicine (MD) from University of Texas, Medical Branch at Galveston, TX. He has been a board certified family medicine physician since 2001.

In addition to general preventative and acute care, Dr. Bannister has extensive OB/GYN experience and specializes in women’s health screenings and visits as well as pediatric care, diabetes care and minor surgery procedures. His focus is on providing patient-centered care and evidence-based medicine to create individualized health plans.

Dr. Bannister is welcoming new patients – newborns through seniors – at our Bedford practice.

Robert Dorf, DO, FAAFP

[Londonderry Office] Dr. Dorf earned his Bachelor of Science degree in Biology from Muhlenberg College, Allentown, PA and his Doctor of Osteopathic Medicine (DO) from University of New England College of Osteopathic Medicine, Biddeford, ME. He has been a board certified family medicine physician since 2001.

He enjoys all aspects of family medicine and has a special interest in pediatrics, geriatrics, musculoskeletal medicine, veteran care, and behavioral health.

Dr. Dorf emphasizes the importance of taking care of the whole person and encourages proactive health and wellness to prevent disease. He says that, “Getting to know people and understanding their world better makes for more effective care.”

In his free time, Dr. Dorf enjoys spending time with his wife and three children. He also enjoys computers, reading, soccer, cars, hiking and skiing.

Adam Joseph, APRN, FNP

[Windham Office] Adam received his Bachelor of Science in Athletic Training from Minnesota State University, and Master of Science in Health and Human Performance from University of Tennessee, Chattanooga. He earned his BSN (Nursing) from University of West Georgia and his Master of Science in Nursing: Family Nurse Practitioner from Duquesne University.

He has a special interest in helping patients with diabetes and in sports medicine.  He encourages his patients to discuss their concerns with him so that he can work with them to create a plan that works for them.

In his free time, Adam enjoys watching football, grilling, exercising, traveling and watching TV.

Douglas Phelan, DO, MPH, FAAFP

[Windham Office] Dr. Phelan completed his undergraduate studies in Biology from La Salle University in Philadelphia, PA. He earned his Doctor of Osteopathic Medicine from Philadelphia College of Osteopathic Medicine in Philadelphia PA and completed his family medicine residency at the NH Dartmouth Family Medicine Residency in Concord, NH. He subsequently completed the Leadership Preventive Medicine Residency at Dartmouth-Hitchcock and holds a Master’s Degree in Public Health (MPH) from The Dartmouth Institute for Health Policy and Clinical Practice.

On behalf of his colleagues and his patients, he is active in organized medicine. Dr. Phelan currently serves (2024-2026) as President of the New Hampshire Academy of Family Physicians.

Dr. Phelan believes that Family Medicine is both science and art. Getting to know you and your family, and understanding your goals, is not only foundational to good care – it’s the best part of the day. He is very comfortable and experienced with patients of all ages, and enthusiastically cares for multiple generations of families from babies on up. He promises to bring the best current medical evidence to bear to make shared decisions that help you to achieve your health goals.

In his spare time, Dr. Phelan is also an avid vocalist and musician with a passion for trivia. When not in the office, he enjoys time with his wife, two young children, and their rescue dog, Chance.
