At DMC Primary Care, we continually strive to make medical care, information, and data available to our patients in the most efficient way possible. You can order prescription refills, get a referral, pay a bill, and find helpful forms directly on our website.

Additionally, our Patient Health Portal is a unique tool that stores your personal patient information — including your contact info and insurance info — in one secure place. Existing account holders can access the Patient Portal here.

If you do not yet have an account, you can set one up either by phone or email:  Send us your email address via an email request, or Call 603.537.1300 and choose option 3. Our receptionist will send you an email invitation to join the Patient Portal.
