WELCOME TO DMC’S PRESCRIPTION RENEWAL PAGE
Maintaining your medications is important to your health. We prioritize convenience and efficiency to ensure that our patients receive medication refills in a timely manner. We have outlined our “best practices” for refills below.
Please note that ALL medication refills require the patient to have at least one appointment in the past 12 months. Some medications require more frequent appointments in order to monitor your safety, medication management, and continuum of care. Please call 603-537-1300 to schedule an appointment or you can self-schedule online HERE.
Here is a guide to help you with the refill process:
Please allow at least THREE BUSINESS DAYS (excluding weekends), for your refill to be processed by our refill team.
- Please contact your pharmacy for a refill/renewal. The pharmacy will reach out to DMC on your behalf.
- You can also request prescription renewals by logging into the FollowMyHealth Patient Health Portal . Please be sure to include your name, date of birth, name of medication and your preferred pharmacy in you request. You will only receive a reply from DMC if we need more information to process your refill request.
- For refills that cannot wait for THREE BUSINESS DAYS, please call 603-537-1300 and speak with a Patient Service Representative.
Please request your renewals at least 3 to 4 days before your prescription is due to run out. We also ask that you remind your provider of any needed renewals during any office visit.
Important to remember:
- Prescriptions must be current to be considered for a renewal. Please call us for an appointment if you need to restart any previous medication.
- If you have not received your refill three business days after your request, please contact your pharmacy for the status of your refill.
- You will not receive a call back from our refill team unless there is a question about your medication renewal.
