ESTABLISHING CARE IS EASY:

Fill out and submit a medical records release form (below).

Call 603-537-1300 to register as a new patient. Please have your insurance card on hand.

Check with your insurance plan to see if they require you to update your Primary Care Physician in their system.

Once registration is complete and we have received your medical records release form, a member of our Patient Service Team will contact you to make an appointment.
