ESTABLISHING CARE IS EASY:

Call 603-537-1300 to register as a new patient. Please have your insurance card on hand.

Fill out and submit a medical records release form HERE.

Check with your insurance plan to see if they require you to update your Primary Care Physician in their system.

Once you have completed registration and your medical records release form, a member of our Patient Service Team will contact you to make an appointment.
